
What to Bring to Your First Appointment
A little preparation can make your first appointment run more smoothly and save you time at reception. We ask that you bring a few things if you have them:
- Photo ID such as a driver’s licence or passport.
- Your health fund card if you have private health insurance.
- A current list of medications you’re taking, including blood thinners, bisphosphonates for osteoporosis, and anything for diabetes or blood pressure.
- Previous dental X-rays taken in the last two years or relating to an existing concern.
- Your referral if your GP or another dentist referred you.
Don’t worry if you can’t track everything down. Bring what you have and our reception team will help you fill in the gaps. Your dentist will assess what information is needed and may recommend new X-rays if something can’t be properly checked by examination alone.
It also helps to let us know whether you’re booking a routine check-up or have a particular dental concern. That way, our reception team can allow appropriate time for your appointment. We can also give you the item numbers to check your rebate directly with your health fund before going ahead with treatment.
What Happens at Your First Appointment
Arrival and Paperwork
When you arrive, our reception team will welcome you and check your details. If you haven’t already completed your new patient form, you can finish any outstanding paperwork in the comfort of our waiting room.
Examination
Dr Thomas Munro or your treating dentist will ask about your dental concerns and examine your teeth, gums and bite. For a dental check-up, this includes checking for signs of tooth decay and gum disease. X-rays are only taken when needed. Let us know if you’re anxious or if a tooth is tender and we’ll go at your pace.
Discussion
Your dentist will talk you through what they’ve found. If treatment or further investigation is recommended, they’ll explain the options and answer your questions. You’ll know what the next step involves before deciding how you’d like to proceed.

Submit an enquiry and a member of our friendly front-of-house team will be in touch to help arrange your appointment. They’ll also set you up with access to your patient portal, where you can complete the required onboarding forms before your visit.
For your first appointment, please arrive 10–15 minutes early to allow time to get settled and help us keep your appointment running on schedule. If you have any questions beforehand, our team is always happy to help.

Getting to Dental 359 Subiaco
Dental 359 is at 359 Rokeby Road, Subiaco WA 6008, near the corner of Heytesbury Road. The clinic is wheelchair accessible.
You can usually find street parking on Rokeby Road or in the surrounding streets. Parking is paid hourly on weekdays and subject to City of Subiaco regulations. There are also paid car parks a 10–15-minute walk away towards the centre of Subiaco.
If you are coming by public transport, Transperth bus route 97 stops directly outside the clinic (Rokeby Rd Before Heytesbury Rd stop), or we are a 16-minute walk from Subiaco Station.
